New Patient Information

What to expect at your first visit:

• When to Arrive:
• For your initial appointment, please arrive 15-20 minutes early to complete all necessary paperwork.
• We encourage you to download the new patient forms listed below and complete them beforehand to expedite the new patient registration process.

• What to Bring:
• Please bring your insurance card and your doctor's orders for physical therapy, if one was given to you.
• Your paperwork (if you downloaded it beforehand)
• Your calendar/ day planner to schedule follow-up appointments
• What to Wear:
• Please dress with loose, comfortable clothes appropriate for your injury (i.e., shorts or loose pants for knee injuries) and comfortable shoes.
